I, the undersigned, am the parent or legal guardian of the minor child named below (hereinafter referred to as "the Volunteer"). I hereby give my full consent for the Volunteer to participate in volunteer activities organized by Gathering Outreach Community Services (hereinafter referred to as "GOCS"). I understand and acknowledge that participation in such activities is entirely voluntary and may involve physical activities, interaction with other volunteers, and exposure to various environments, which may carry certain inherent risks.
Release of Liability and Hold Harmless Agreement:
In consideration of GOCS allowing the Volunteer to participate in its programs, I, on behalf of myself, the Volunteer, and our respective heirs, executors, administrators, legal representatives, and assigns, hereby release, waive, discharge, and hold harmless GOCS, its officers, directors, employees, agents, volunteers, and affiliates from any and all liability, claims, demands, causes of action, or expenses (including reasonable attorneys' fees) of any kind or nature, including but not limited to damages, loss, injuries, or death, that may arise from or in connection with the Volunteer’s participation in any activities or programs conducted by or associated with GOCS.
Acknowledgment of Risk:
I acknowledge that I have been informed of the nature of the activities the Volunteer may be engaged in and that these activities may involve certain risks, including but not limited to the risk of injury, illness, or death. I understand that GOCS cannot and does not assume responsibility for any such risks and that I am responsible for assessing the appropriateness of these activities for the Volunteer.
Medical Authorization:
In the event of an emergency, I authorize GOCS to obtain necessary medical treatment for the Volunteer and agree to be responsible for any costs incurred due to such treatment.
Indemnification:
I agree to indemnify and hold harmless GOCS, its officers, directors, employees, agents, volunteers, and affiliates from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees brought as a result of the Volunteer’s participation in GOCS activities and to reimburse them for any such expenses incurred.
Consent to Media Release:
I consent to the use of the Volunteer’s image, voice, or likeness in any photographs, videos, or other media taken during GOCS activities for promotional, educational, or other purposes without compensation.
